 CENTURY SIM RACING - CERTIFICATE OF UNDERSTANDING AND EXPRESS ASSUMPTION OF RISK ALL PARTICIPANTS MUST READ THIS FORM CAREFULLY.

I hereby confirm that I am in good physical and mental health, my eyesight (as corrected as appropriate) is up to the standard required for a road driving test and I am not suffering from any other medical condition or disability which might make it unsafe for me to participate. I further state that I am not participating against medical advice and that I do not have any pre-existing condition, which might make it unsafe for me to participate. I further state that it is my responsibility to make sure that I treat all the equipment at my disposal with the care necessary to ensure that any undue harm does not befall either myself or the equipment. I further state that I am happy to have my name and telemetry data shown on the Century Sim Racing website.

By my signature I accept that I fully understand that the terms of this document and the reasons why I have been asked to sign it and hereby confirm that I am signing it of my own free will. 

I further state that I am of lawful age and legally competent to sign this certificate and that I have acquired the written consent of my parent or guardian as appropriate. confirm that I am 16 years of age or over.
